Abstract Background Iatrogenic perforation is arguably the most feared adverse event associated with endoscopy. Current American Society for Gastrointestinal Endoscopy (ASGE) and European of Gastroenterology (ESGE) guidelines recommend endoscopic closure as first-line treatment strategy. Historically, this has been achieved using through-the-scope clips (TTSC). Given emergence alternative techniques including over-the-scope (OTSC) suturing, we sought to provide an updated review literature. Purpose To following iatrogenic during screening or therapeutic Method Based on Preferred Reporting Items Systematic Reviews Meta-analyses (PRISMA) Guidelines, electronic search MEDLINE EMBASE from June 1st, 1946 – Oct 10th, 2022 was performed. Inclusion criteria limited English full-text original citations, case reports, cohorts < 3 patients excluded. Our primary objective assess complete defect after attempted treatment. Outcomes were stratified by modality (TTSC, OTSC, suturing) chronologically based a previous well received systematic review. Result(s) A total 2549 citations identified in our search, which 34 included representing 830 perforations. Overall, successful 763 cases (91.9%). When techniques, range estimates 71% 100%, 57% - 100% TTSC, OTSC suturing respectively. stratifying chronologically, improvement TTSC identified. Conclusion(s) Endoscopic closure, are effective management perforations increasing performance over time. It remains strategy perforation.
